Members Living Within the State of Louisiana

If you are insured under an HMO, POS, High Deductible, or Health Assurance product, then your Participating Provider network includes providers located within the State of Louisiana that have contracted with Coventry Health Care of Louisiana, Inc. To receive the maximum benefit available under your Plan, you must receive care from a Coventry Health Care of Louisiana, Inc. provider. Please follow the link below for a list of providers.

Members Living Outside of the State of Louisiana

If you live outside of the State of Louisiana and are insured under a PPO product, or if you are a full-time student living outside of the State of Louisiana and have provided acceptable proof of your status to Coventry Health Care of Louisiana, Inc., then your Participating Provider network includes providers that have contracted with the Coventry Health Care National Network. To receive the maximum benefit available under your Plan, you must receive care from a Coventry Health Care National Network provider. Please follow the link below for a list of providers.
